If your child is still in need of a device, please select the link below to fill out the device request form.

You will be notified of the time and date for pick up. We also ask, if possible, please fill the Parent and Student Notification regarding rules concerning the use of loaned devices (i.e. tablets, laptops) and related accessories assigned to students and bring it to school with you. If you are unable to print it, we will have forms available. We thank you for your patience during this process.
